If yes, you will be prompted to provide information regarding your Maestro project number (M#) and/or IRB/AUP/IBC protocol number (if applicable). If not funded, use the space provided to describe.
2. Are the expenses for this travel being paid for or reimbursed by a foreign entity or individual?
If so, please ensure that you include the full name and associated entity for the individual (and/or entity name for a foreign entity) in your response.
3. Do you plan to meet or collaborate with any individuals during your travel?
NOTE: For the purposes of this review, you should not include individuals you may casually interact with as part of networking during a conference. Per System Regulation 15.05.04, a high risk international collaboration includes: sharing of research facilities, resources, or data; sharing of technical know-how; any financial or in-kind contribution intended to produce a research product; sponsorship or facilitation of research fellowships, visas, or residence permits; joint ventures, partnerships, or other formalized agreements for the purpose of conducting research or sharing resources, data, or technology; inclusion of researchers as consultants, advisors, or members of advisory or review boards; and such other activities as may be determined by the Secretary of Defense (as outlined in Section 238(d)(1) of Public Law 118-159).

NOTE: This can include information/data subject to intellectual property restrictions, under publication restriction, is classified, is proprietary, and/or is unpublished.

If yes, please list and be able to provide the information required in the International Shipping Request form.
6. Will you be taking/shipping chemicals, biologicals, composites, or other materials?
If yes, please list and be able to provide the information required in the International Shipping Request form.
7. Are you taking an institutionally owned laptop?
If yes, please contact your IT department for assistance with obtaining a loaner laptop for your trip.

8. Do any electronic devices you are taking contain controlled software, technology, or information OR have advanced or unique computing capabilities?
NOTE: Typically, this does NOT include off-the-shelf laptops with standard operating systems, basic administrative software packages, and mass-market encryption capabilities. However, it could include equipment/software that are customized for use with a particular instrument, containing specialized research software, etc.
9. Are you a Responsible Party under a current Technology Control Plan?
If you have research or equipment controlled on a Technology Control Plan, you would have a signed TCP with the Research Security and Export Controls office and would routinely be in contact with our office regarding this information or equipment.
